It is important that we have your correct address, phone number, and email.
This helps us contact you about appointments, test results, and your care.
Change of address
If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.
Check you are within our catchment area.
You must visit reception in person and bring;
- Photo ID
- Proof of new address
Changing your phone number or email address
You will need to visit reception and show us photo ID before we can change your phone number or email address.
Changing your date of birth or name
You will need to visit reception and show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
